Summary of Services

FAMILY HEALTH CARE

*** All co-pays and upfront fees are due at time of service. The remainder of the fee is due upon discharge.

*** Quality of Life accepts Medicaid, Medicare and most private insurances. Discounts are available for uninsured patients upon eligibility approval (See Patient Information).

LAB and X-RAY SERVICES

Lab services are available at all sites within the QOLHS network. X-ray services are available at the Quality of Life Health Complex, Anniston Quality Health Care and Roberta O. Watts Medical Center

DENTAL SERVICES

Dental services are available at the QOLHS Health Complex, Anniston Quality Health Care and Talladega Quality Health Care. Patients may visit any of these sites. Payment is due at time of service.

PHARMACY SERVICES

The QOLHS Pharmacy is located inside the QOLHS Health Complex and the Roberta O. Watts Medical Center for your convenience. The QOLHS Health Complex will not fill prescriptions from other physicians or doctor’s offices outside of the QOLHS network without a QOLHS provider’s signature.  The Roberta O. Watts Medical Center Pharmacy is open to the public. 

PRESCRIPTION ASSISTANCE PROGRAM

A nominal fee of $15.00 is required for each medication received through the Prescription Assistance Program. Eligible patients receive up to a 3-month supply of most medications approved under the program. Prescriptions, income verification and tax information (if applicable) is required in determining eligibility.

EYE CARE SERVICES

Eye care services are available at the Quality of Life Health Complex, Anniston Quality Health Care and the Roberta O. Watts Medical Center. Medicaid is accepted for exams and glasses. Medicare accepted for exams only. Other commercial insurance (with vision component) accepted.

MEDICAID SERVICES

QOLHS houses two Medicaid case workers within the Special Services Department.

PROCARE MEDICAL CENTER

There Is no Co-Pay for Public Housing Residents and Points within a one (1) mile radius of any Public Housing at ProCare ONLY

SLIDING-FEE DISCOUNT PROGRAM

Uninsured patients may be eligible to receive reduced fees through the Sliding-Fee Discount Program. Discounts range from 25% to 75%, depending on the financial situation of the patient. Patients applying for reduced fees must provide written documentation of source of income and proof of residence at the time of service.

Patients must provide at least one of the following documents as proof of income:

  • Check Stub dated 6 months or less

  • Food Stamp Letter

  • Social Security Letter

  • Medicaid Card

  • Medicare Card

  • Unemployment Letter

  • Current W-2 Form

  • Bank Statements

  • VA

  • Child Support

Patients must provide at least one of the following as proof of residency:

  • Mail addressed to patient

  • Utility bills addressed to patient

Once approved for the sliding-fee, patients will be given a patient identification card which will allow them to receive discounts on pharmacy, lab and x-ray services.

PHARMACY DISCOUNT

The QOLHS Pharmacy is located inside the QOLHS Health Complex and at the Roberta O. Watts Medical Center for your convenience. Patients can receive up to 75% off of medications with use of the Patient Identification Card.
